**Please note that a puppy application must be on file and approved prior to acceptance on any Midwest Chihuahuas waiting list and prior to any deposits or payments will be accepted.  Midwest Chihuahuas puppies, particularly in the merle, brindle and blue colors find homes particularly fast.  If you are interested in any of these colors, please do not delay in submitting your application so that you may be placed on the waiting list of your choice or be allowed to put a deposit on an available puppy to hold until age appropriate to leave.**

This application should be filled out in it's entirety.  We ask each prospective family to answer all questions openly and honestly so that we can offer the best help in attaining the puppy of your dreams.
